  A Dallas family of an 18-year-old boy who died after being treated with a tainted IV
  bag is furious after being handed a $12,000 bill from the hospital.

  After Jack's passing his family was told the facility 'waived 100 percent
  of your son's bill,' Jack's father, who was not identified, told . 
  
  Although the family anticipated still receiving
  a bill, as most medical bills are automated, Jack's father
  had emailed the facility five days after Jack's surgery to 'confirm that neither I nor our insurer is going to receive a bill.' 
  
  'I will be extremely displeased if I do. I recognize that sometimes billing is on autopilot,' the father wrote in an email. 
  
  But to add 'salt to the wound,' they received a bill for $12,903, which has since been taken care of by

  Dr. Kasper's autopsy revealed she had died from a lethal dose
  of bupivacaine, a drug that is 'rarely abused,

  None of the incidents occurred during Ortiz's surgeries.
  All the incidents occurred while Ortiz was inside the facility and none occurred while he was on vacation,
  the Northern District of Texas said.

  Ortiz was notified of a disciplinary inquiry stemming from an incident during which he allegedly

  concealing an IV bag in a paper folder after he came out of surgery and putting the bag
  into the warmer and swapping it for another.
  A half-hour later, a 54-year-old woman suffered an emergency during her scheduled cosmetic surgery. 
  
  Another time, Ortiz was reportedly seen on video suspicious placing the IV bag in the warmer before
  scanning the hallway and 'quickly' walking away. An hour later, a 56-year-old suffered a cardiac emergency during her cosmetic surgery. 
  
  Ortiz reportedly had a history of disciplinary actions against him,
  which the doctor claimed was the center trying to 'crucify'

  US Attorney Chad E. Meacham said the office will 'vigorously prosecute this case.' 
  
  'A single incident of seemingly intentional patient harm would be disconcerting;
  multiple incidents are truly disturbing,' he said. 
  
  'Patients expect that their doctors will use only safe and
  effective medical products during their surgeries,'  FDA
  Office of Criminal Investigations, Special Agent in Charge Charles
  L. Grinstead, said. 
  
  He has also been accused by an ex-girlfriend, who is
  a nurse, of drugging her, but a police report was never filed, according to He also reportedly owes
